1. The administrator of your personal data is Myrhorod sp z o.o based in Gdynia (81-310) Sląska 50 Street, NIP 5862360584, hereinafter referred to as “Administrator”,
2. Your personal data is necessary for us to: carry out the tattoo, prepare a health questionnaire to assess the admissibility of the tattoo, contact you to book an appointment, fulfill tax and accounting obligations, defend and assert claims,
3. The basis for processing your personal data is:
4. 1.Article 6(1)(b) of Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data and repealing Directive 95/46/EC, hereinafter referred to as RODO,2.Article 6(1)(c) of the RODO,3.Article 6(1)(f) RODO – as to the assertion of possible claims in connection with the performance of the service and the defense against claims,4.Article 9(2)(a) RODO – as to data concerning your health.
5. The provision of data is voluntary, however, without the data we will not be able to perform the tattoo for you,

You have the right to:1. request Us to access, rectify, erase or restrict the processing of your personal data,2.to object to such processing, or to data portability,3.to lodge a complaint with a supervisory authority,4.to withdraw your consent to the processing of your personal data where the processing of your data is based on consent,5.request to receive one copy of the personal data processed by Us,
7.  Your personal data will not be transferred to recipients of your personal data, ta to third countries and international organizations,
8.  Your personal data will not be subject to automated decision-making, including profiling,
9. Your personal data will be stored for no longer than 5 years, starting from the beginning of the calendar year following the year in which you last used the services of Machete Tattoo Gdynia
